Currently the District is asking all Phase IV customers to complete an easement with a Notary and send to the District Office.
Note: The information on the first line of the easement should list the ownership of property exactly how it appears on the deed. (Sample: Doe, John S. & Jane R.)

Note: All legal property owners listed on the deed must sign.

Note: The Deed Book & Page number/Instrument number will be researched; you may leave it blank if you do not have this information.
If you live in Liberty Township please complete the Easement.
If you live in Monon Township please complete the Easement.

Currently the District is asking all Phase IV customers to complete an Information Sheet and send to the District office.

Phase III & IIIB ***NOTICE***DEADLINE EXTENSION***

Permits purchased after October 12, 2009 will cost $ 100.00

In an effort to coordinate the completion of all Qualified Contractor connections, the District is extending permit deadlines for CONTRACTOR INSTALLATIONS ONLY until August 15, 2010.

If a permit HAS NOT been purchased and a Qualified Contractor will be used, you will need to provide written confirmation from your Qualified Contractor on their company letterhead stating confirmation of work to be completed and your service address.

ALL HOMEOWNER CONNECTIONS must be completed by May 17, 2010.
